Shoulder injuries are one of the most common categories of WorkCover Queensland claims, and one of the most frustrating to recover from. The shoulder is the most mobile joint in the body, which is exactly why it is vulnerable: overhead work, repetitive lifting, sustained awkward postures and sudden loads all take their toll. Whether your claim involves a rotator cuff strain, bursitis, impingement or a post-surgical recovery, the path back to full function follows recognisable stages - and the quality of the rehabilitation program determines how well you travel them.

Why work-related shoulder injuries linger

The shoulder trades stability for mobility. Its function depends on a coordinated system: the rotator cuff muscles that centre the joint, the larger movers like the deltoid and lats, and the muscles that control the shoulder blade. When one part of that system is injured, the others compensate - and compensation patterns can become entrenched quickly. Add the natural instinct to protect a painful shoulder by not using it, and you have the recipe for stiffness, weakness and a recovery that drags.

For workers whose jobs involve lifting, carrying or overhead tasks, there is an additional challenge: the bar for recovered is high. It is not enough for the shoulder to feel okay at rest. It has to tolerate load, repetition and duration - shift after shift.

Stage 1: Settle, protect and assess

Early rehabilitation focuses on settling irritation, maintaining the movement you have, and building an accurate baseline. At Optimum Movement Centre this begins with a detailed physiotherapy assessment: which structures are involved, which movements provoke symptoms, how the shoulder blade is moving, and what your job actually demands of the joint. Where surgery has been involved, we work within your surgeon's protocol and timelines.

Baseline measurement matters just as much for shoulders as for backs. Objective strength testing of the shoulder and upper limb gives your claim a documented starting point and gives the program a clear target.

Stage 2: Restore the foundations

The middle stage of shoulder rehabilitation is about rebuilding the joint's support system in the right order:

  • Rotator cuff strengthening - controlled, progressive loading of the muscles that stabilise the joint, at ranges and intensities that increase as tolerance improves.
  • Scapular control - retraining the muscles that position the shoulder blade, because a poorly controlled scapula undermines everything above it.
  • Range of motion - graduated work to restore movement without repeatedly flaring the injured structures.
  • Kinetic chain work - the trunk, hips and legs contribute to almost every lifting task, so they are trained alongside the shoulder rather than ignored.

Our facility in Varsity Lakes is purpose-built for this kind of precise, progressive loading. As Australia's only accredited MedX Medical facility, we use medically oriented equipment designed to apply smooth, controlled resistance through a full range of motion - allowing the injured structures to be loaded accurately rather than approximately. Progress is measured at intervals, so improvement is documented rather than assumed.

Stage 3: Rebuild work capacity

The final stage bridges the gap between clinical strength and job demands. For a shoulder claim, that typically means progressive exposure to the specific tasks your role requires: lifting to bench height, then overhead; carrying; pushing and pulling; sustained or repetitive tasks built up in duration. This stage runs hand-in-hand with your suitable duties plan, so the tasks you are cleared for at work track with the capacity you have demonstrated in the program.

Throughout, your treating team should be communicating with your GP and your WorkCover case manager so that work capacity certificates reflect your actual measured capacity - not guesswork.

Your rights as an injured worker in Queensland

Two things every Queensland worker with a shoulder claim should know. First, physiotherapy and structured rehabilitation for your accepted injury are covered by WorkCover as reasonable treatment, with providers requesting approval for sessions as the program progresses. Second, you have the right to choose your own treating physiotherapist - including changing providers if your current program has stalled.

At Optimum Movement Centre we bill WorkCover Queensland directly for approved treatment, so there is no upfront cost juggling for approved sessions on an accepted claim. Bring your claim number and current work capacity certificate to your first appointment and we will take it from there.

Common mistakes that slow shoulder recovery

Three patterns show up again and again in stalled shoulder claims. First, resting the shoulder completely for too long - protection is appropriate early, but extended disuse breeds stiffness and weakness. Second, returning to full duties on feel rather than measured capacity, which invites re-aggravation and a longer claim. Third, training around the shoulder rather than through it - endless general fitness work while the rotator cuff itself never gets progressively loaded. A structured program is designed to avoid all three.
